Reimbursement structures have reshaped transitional healthcare facilities which include rapid recovery, subacute care, rehabilitation, memory care, substance abuse, behavioral health, hospice, and senior living. Each of these environments is unique, but all share a need for major transformation. These settings vary from small residential care facilities to large Continuing Care Communities that include all levels from independent living to skilled nursing services. Lead by the care for our aging population, this group desires a departure from the institutional approach of the past toward more creative designs. Resident centered care models used by the senior healthcare industry promotes personal dignity, unites caregivers, and leads to better outcomes.
